Event liability waiver

By attending the Mama Hub playgroups, I agree that I and my child(ren)/ward(s) are voluntarily participating in the activities offered by Mama Hub. I am assuming on behalf of myself and/or child/ward, all risk of personal injury, death, or disability to myself and/or child(ren)/ward(s) that may result from participation in the Mama Hub playgroup, or any damage, loss or theft of any personal property which me and/or child(ren)/ward(s) may incur.

I expressly release, hold harmless, indemnify and defend owner and employees, and agents of Mama Hub LLC from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of injury, loss or damage to my or my child(ren)/ward(s), howsoever caused, arising by reason of or during the child(ren)’s participation in the Mama Hub LLC playgroups and/or activities.

I understand that there may be photographers at the Mama Hub events and that my/my child(ren) and/or ward(s) image may be captured and used in promotional materials and that if I do not agree to this, I must alert an agent of Mama Hub LLC.

Terms of service:

We understand that, when you have young children, unforeseen circumstances can arise that mean you might not be able to attend the playgroup. In the event that you are unable to attend, please notify Mama Hub and we will be happy to roll your booking across to the next available booking. Please make sure you notify Mama Hub, as we will be unable to rollover bookings unless contacted within a 48 hours. Once booked, we are unable to issue refunds.
